Archaeological Report Inventory Form (ARI)
(PDF, 61 KB)- The ARI form should be included in the final report to facilitate report processing and Wisconsin Historic Preservation Database (WHPD) updates.
Archaeological Site Inventory Form (ASI)
(PDF, 72 KB)- The ASI forms should be submitted prior to sending the report in order to assist the state in compiling statewide data.
