NOTES
Additional Information:Re-surveyed for 2010 DOT survey: Erected in 1901 to house the Farmer's Store (originally established in West Salem in 1892). The general dry goods store was "well-stocked" for a small town like West Salem but after the rise of the automobile, declined in popularity. The store was closed in 1968. Today it houses a restaurant.

Previously surveyed in 1992.
