NOTES
Additional Information:Physician Abram Levine constructed this building sometime between 1955 and 1960.

2015- "This one-story, Contemporary-style physician's office is faced with brick and topped with a flat roof. The main (north) facade retains a recessed doorway flanked on the left (east) side by a line of four plate-glass windows underlined with either hopper or awning windows; a brick planter is located underneath. A four-light corner window unit appears to the door's right. Three-light casement window units also pierce the main facade. A sign on the eastern facade identifies the building as the Greenfield Medical Center.

Physician Abram Levine constructed this building in 1957. It was designed by Frederick J. Schweitzer. It was taken over in 1977 by Dr. J.S. Kostrzewski and became known as the Greenfield Medical Center."
