NOTES
Additional Information:2011- "This 1-story Ranch-style medical office was constructed in 1966. The building's northwest elevation is attached to a large
medical center of concrete and metal composition of later construction. The medical office has a concrete foundation, brick, glass, and T1-11 walls, and an asphalt-shingled side-gabled roof. The front elevation of the complex faces northeast; the medical office is asymmetrical in plan with a projecting flat roof over a recessed entry that spans the southeast half of the building. The flat roof over the entry terminates in a peak that creates a front-gabled vestibule with glass walls. Small fixed-pane windows are located at intervals just below the roofline of the medical office."
