2010: This two-and-one-half-story, stucco-clad Arts and Crafts-style former house consists of a clipped, front-gabled block with gabled and hipped wings. The building features a slate roof with exposed rafter tails or decorative beams under the eaves. A projecting, one story entrance block has a gable-on-hip roof. Fenestration consists of wooden, double-hung windows with leaded, multiple-light upper sash over single-light lower sash. Other notable elements include two-story canted window bays and raised stucco-covered bands.

Originally constructed in 1881, the C.D. Booth residence was remodeled significantly by architect H.W. Buemming and builder Forster Construction Company in 1910. Real estate manager, E.A. Bauer, was listed as the property owner at time. In 1947, the building was again remodeled as a doctor's office for Raymond Warner. Dr. Warner maintained his eye practice at the building until at least 1971. In 1973, Now Optical resided at the location.
