Pen-and-ink on paper. Includes certifications signed by surveyor and by "John T. Perkins, by his attorney I.A. Lapham." Inscribed and signed certification on verso by a notary public of Milwaukee County. Top reads: "I hereby certify that I have surveyed the five acres of land described in the accompanying certificate, and have subdivided the same into Blocks, Lots, Streets and an Alley on the courses and of the dimensions set forth on this plat. Milwaukee January 21st 1855 Joseph Clarke Deputy Surveyor 2nd Ward Milwaukee". Bottom reads: "Perkins' Addition to the City of Milwaukee This is to certify that I have laid out a certain tract of land in the Second Ward of the City of Milwaukee, beginning on the south line of the north west quarter of section twenty (20) town seven (7) range twenty two (22) three chains west from the centre of said section twenty, and running thence north sixteen and two thirds chains; thence east three chains to the east line of said quarter section; thence north three and one third chains; thence west five chains; thence south twenty chains; thence east two chains to the place of beginning (continuing five acres), into Blocks, Lots, Streets, and an Alley, as represented on this plat, to be known as Perkins' Addition to the City of Milwaukee; and that the course of the streets and alley, their dimensions, and the dimensions of the lots (in feet) are as shown on the plat. In written whereof I have hereunto set my hand and seal on this 21st day of July A.D. 1855 John T. Perkins By his attorney I.A. Lapham In presence of Charles F. Bode." Written in what appears to be pencil over this certification is: "This plate Cancelled".
