Text on reverse reads: "All Faiths Chapel. Winnebago State Hospital. Winnebago, Wisconsin. Completed in 1969. In addition to the main chapel, which seats 200, it has two smaller chapels, offices for the hospital chaplain's and several meeting rooms. This chapel was used by all faiths." Additional text on a sticker on the reverse reads: "Friends of Julaine Farrow Museum. To establish and develop effective community partnerships and promote awareness of the Julaine Farrow Museum; Located on the grounds of Winnebago Mental Health Institute. The friends will assist with working with museum archives and artifacts; communicating and promoting the museum within their communities, while supporting WMHI, its mission, its past, and its current needs. The friends will assist in coordination the efforts of groups and individuals interested in the welfare of historic preservation." The chapel is surrounded by a lawn and trees.
