Additional Information:Front and two side roof dormers with leaded glass windows; rectangular two story bay on north side and three sided two story bay on south side; flat arched windows with brick lintels. Full length dentil trimmed open porch with short Corinthian columns on brick bases; high stone foundation. Identical to 40-34.

Constructed by the Kutzborsky family associated with the Segelke-Kohlhaus Company, this house and the identical adjacent house at 729 South 5th Avenue are well preserved architecturally significant representatives of the American Foursquare house. Houses built by brothers Charles and John Kutzborsky.

2017 UPDATE - THIS BUILDING IS SUBSTANTIALLY THE SAME AS WHEN IT WAS LAST SURVEYED IN 1996.
